As someone who has found joy in the Permadeath mode, I’m a bit saddened no npcs really recognize the feats the lamb is proving by not dying. The one thing that always sticks out to me is Kallamar. When you get to the third crusade, they talk about how neither illness or death stops you and laments in their inevitable end. It would be so cool if it was changed to Kallamar recognizing that you’ve never fallen and seem hesitant to die. Instead of being scared at inevitability, being scared at your sheer power.

Also, some sort of reward for beating a game on permadeath would be cool to you know. But I’m not like totally salty and crap that I beat Narider and I don’t even have a special permadeath statue or anything to show bragging rights. I… I swear I’m not… it would be cool tho.

Kallamars line really bothers me - not just permadeath mode, but if anyone manages to get to him without dying before, he should acknowledge it.

he might mean "death nor illness" in the more... general sense
like "the forces of death (the armies of old faith followers, the illnesses I send, even the bishops themselves) cannot stop you"

I agree alternate dialogue would be cool but I think the original dialogue still works

“It seems you cannot be stopped by disease or hunger. And he sends you back from death stronger each time”
This is an exact quote, I’m sorry.

They should probably just have alternative dialogue
