Also nothing in here is official and just theories and some applies physics so 
#1: Dark Matter
Dark matter is a particle that is not comparable to other normal particles. It is firstly very dense, and is produced in varying amounts depending on the speed of a hadron collider. It has to be stored in special fuel cells in order to keep it at a set temperature and stable. Unlike normal materials, dark matter does not change states, instead it decreases in size the higher the temperature, with useful changes beginning at temperatures above 500 degrees kelvin (with some fluctuations present), and major stability starting at 1000k. Because of these properties, dark matter is extremely useful in facilitating fusion reactions at lower temperatures, due to the fact that as it’s size decreases it begins affecting a greater pull on other particles, which can be utilized to help overcome the charges that repel tritium and deuterium. However, the dark matter once out of the fuel cell will slowly decay and fuse with loose neutrons, electrons, and positrons to form heavy elements.
#2: Fuel cells
The fuel cells were originally designed to only contain the dark matter that was produced and keep it stable. However, it was quickly found to be a better idea to premix the Dark matter, tritium, and deuterium as they wouldn’t react with the dark matter. As certain mixture amounts of dark matter, deuterium, and tritium were made, three different kinds of fuel cell mixes were designed. The first, called reactive, consisted of relatively larger amount of dark matter, with smaller amounts of deuterium and tritium, which led to the fuel cell rapidly heating the DMR, but also depleting extremely quickly, also trying to produce the correct ratios of fuel for this cell required to push the hadron collider to near overheat which led to these cells not being used often. The second, called normal, was the optimal combination for everyday use, with the ideal ratio of dark matter to tritium and deuterium, and a mediocre rate of depletion and heat generation. The final one, dubbed “efficient”, had relatively lower amounts of dark matter and higher amounts of deuterium and tritium, this gave the cell a slow rate of depletion but also generated much less heat than that of the other cells.
#3: Order of meltdown (E represents 10)
Phase 1: Salvageable Thermal runaway: Reactor has gotten to a temperature such that turning off lasers and putting coolant to max will not stop the reaction, emergency EFSS and relief valves are needed.
Phase 2: Uncontrollable Thermal Runaway: No on-site thermal control systems can bring down the temperature.
Phase 3: Full Runaway, aka Point of No Return: The amount of Dark Matter in the chamber and temperature of the chamber has reached a high enough number such ejection of the fuel cells will not stop a meltdown
Phase 4: Extreme compression of the materials within the internal combustion chamber is occurring due to the high temperatures of the Dark Matter and increasing attracting force.
Event 1: Fuel cell 3’s injection chamber fully fails (a known failure by Kaiser, it had been showing more fuel than there was in the cell and injecting more than it should, leading to the disaster), leading to all remaining fuel within it being pulled into the combustion chamber, this causes a sudden spike of attraction within the combustion chamber, causing all other fuel cell combustion chambers to do the same.
Phase 5: Criticality: The internal combustion chamber’s materials are approaching the schwarzchild radius.
Phase 6: Formation: The attraction of the center of the internal combution chamber causes the DMR to collapse inward and the chamber to form a singularity within 2.56796 E-34 seconds.
Phase 7: Lifetime: The singularity begins evaporating massive amounts of radiation while creating a time distortion.
Phase 8: Evaporation: 1.67149E-12 seconds after it formed, the black hole fully evaporates, emitting massive amounts of excited particles, excited photons, and radiation.
Phase 9: Detonation: Within 2.3349E-8 seconds, the photons make contact with the air, causing an explosion and ensuing shockwave with the power of 2-3 tsar bombs and 10-15 megatons of tnt.

Phase 10: Damage: The ground above the DMR explodes out, creating openings in the containment dome, the reacops cavern collapses inward allowing some of the explosion to enter the sky, making it look like day for a second for a few miles, after which allowing radioactive gases to escape.
Phase 11: Rescue overview: Luckily winds that day would be very low, limiting the spread of fallout and quick response from National Guard and Quantum Services and a very technical response of radiation suits, winches, skycranes(large load helicopters), and local construction workers who volunteered their equipment to help fill in the holes which were allowing radioactive gases to spew out (teams would rescue those in the shelters first, which would be hastened by the use of two prototype quantum exosuits)
Math of a realistic DMR blackhole
The mass of the black hole (how much mass it absorbed), would be .033 metric tons.

Its effective density would be 6.69205 times E to the power of 73 g/cm^3.

It’s time to singularity (how long the matter compressed at its diameter will take to form a singularity) would be 2.56796 times E to the power of negative 34 seconds.
Its entropy would be 2.88886 times E to the power of 19 (dimensionless).
Its temperature would be 3.71805 times E to the power of 21 degrees Kelvin.
Its peak photons would be 1.25618 times E to the power of 18 eV.
Its nominal luminosity would be 3.27084 times E to the power of 29 watts.
It’s lifetime(how long before it fully evaporated/collapsed) would be 1.67149 times E to the power of -12 seconds.
It would have the total energy output of 11 tsar bombs, though most of this would be in the form of radiation, with the rest as excited particles and extremely excited photons. The estimated explosion from the photons contact with the air (based on the survival of the containment dome) would be about 2-3 tsar bombs worth, 100-150 megatons of TNT equivalent.
