For me, the default state is off. And when All input_bollean are off, if I restart HA, when it comes back, one of them is on. On the log I can see it has been turned on by supervisor.
If you set a valid value for initial: , the input_boolean helper will start with the state set to that value. Otherwise, it will restore the state it had prior to Home Assistant stopping.

You can use this to do something, like turn off a group of helper booleans, when HA starts
But as far as I know, I can't set a initial value from the UI. And the helper has been set from the UI
