ah, thats happens all the time, let me explain how:

when the entity is moving, its "updating" its hitbox every severTick level its currently at. (default = 3) the entity was moving towards the door at the exact moment the door was placed at the exact same tick or after that tick, causing the block to disappear, and going back in your inventory. however, this is an iron door, which means it has technically 2 blocks, upper and lower. if you mine the upper door without a bottom, you get a full iron door, basically causing a dupe glitch. minecraft cannot fix this as they have to rewrite the entire tick system along with entityTicks.
