Just one small thing, make sure that mappings/textures for these sorts of things are done within the player item section (middle bottom part of the texture). For something like this, you don't even really need that large of a color pattern; 2 squares of a few pixels each are more than enough.
Also just FYI, historically the game hasn't particularly encouraged bad habits (to include smoking) in an attempt to be more of a family/child-friendly environment compared to other games out there. Even though this leans towards a historical context, I could see it being in a bit of a gray area. Just my 2¢.
Yeah, that is why I stayed away from modern cigarettes as these old ones were considered status displays for Generals as normal troops couldn't always find some. Take Winston Churchill for an example
Should I downscale my image of the cigar textures to fit inside the grey part of the player skin thing?

Yes. Rest of the image can even be transparent so as to make it easier to overlay on another (parent) base texture.

So I insert it in the grey area and delete everything except the texture, but keep the image dimensions and stuff?
That gray area is the limit for any items, so as long as you fit it anywhere inside that area it's fine.

I don't think that's a very valid argument 
It's an fps, there's only so much you can do while working within a given genre. But the low-poly abstraction + lack of gore goes a long way.

I agree about the bad habits and stuff, that is why I stayed away from modern cigarettes, but take your average old Disney Movie for instance. There were people smoking in there and it is a kids film. At least I am not trying to make it look like a modern cigarette. Most kids won't even recognize this old one from 1800s and early 1900s

these days governments heavily reject it via means like heavy taxation
rating boards increase the ratings of things for including various addictive substances even if there's some violence
WB with the right amount and portrayal of various substances would become a higher age rated game
good luck finding smoking in any newer kids films because of the change in attitude (as the studies came in to show just how toxic they are to the lungs and the increased cancer risks and so on)
