The actual dungeon entrance is the dark square on the map, several tiles east of the dungeon icon and in an entirely different map chunk.

I don’t really think it’s a big issue considering you can see the physical ladder on the map and the icon is directly next to it. The “chunk borders” don’t really matter since they’re not a part of normal gameplay or map experience
