Bosses and some enemies have "challenges" associated with them. Something like "Defeat a hellhound" for example.

Each tier request so many points to complete and challenges, or CAs, reward varying amount of points.

I don't think you should be changing your setups too often tbh. There are some breakpoints at bosses where some gear overtakes but it should be very often
