I know that time moves forward in according to how it was originally supposed to, but Diavolo is able to freely move around. The only inconsistent thing is weather or not he is physical in the time gone by.
Sometimes he exists totally outside of physical reality and bullets move straight through them, other times he has to actually get out of the way of projectiles and can interact with other physical things, like the blood droplets thing.
Other times he waits just for his ability end before attacking.
It's one of those "it works like this, except when it doesn't" abilities. Still, I think its rad.
