If you see a "0 Health" bar below NPC names, that was added by a server setting or external plugin, not Citizens. "TAB" is the plugin that most commonly does this. For more information, refer to the FAQ page: https://wiki.citizensnpcs.co/Frequently_Asked_Questions#There.27s_a_.220_health.22_line_below_my_NPC.27s_names.
