I wanted to do it with Citizens but I was told probably not possible and to ask here for Denizens
Get any item > change its model and texture to whatever 2d image you want > put it on the npcs head

im only counting <50 pixels in height at best in that image
256x256 is super easy to work with though

You can make a custom font in your resource pack with your image, then have it in your NPC's name/hologram
And for that you don't even need Denizen, Citizens supports MiniMessage which you can use for fonts

@icy bridge Hello! Thank you for all the help so far, its been extremely helpful. I've gotten my custom image, and created a resource pack with it, making it a custom font. Now I'm wanting to use the minimessage functionality to add it to my Citizens NPC name, the websites has this syntax as an example: font:myfont:custom_fontUses a custom font from a resource pack</font>
Where exactly would I put the font folder name?
'font folder name' is kinda vague, i'd recommend following the guide or a tutorial and basing your usage on that experience
there can be multiple folders and subfolders associated with fonts
