Based on your export options it looks like you made a model in the "Generic Model" format, which cannot be used for Minecraft.
you can try to use File > Convert Project to switch to the correct format, but depending on that format's restrictions the model will look different. (reset rotations, adjusted sizes, removal of mesh elements etc.)
