One thing I have noticed in the past is that, if you complete a mission in multiplayer, it shows up in the singleplayer list as well. Could it be that you played Dealership multiplayer and haven't completed Rust Belt yet?

In my personal opinion, Medal of Valor is harder than The Hanged Man, my reason why is pretty simple

in The Hanged Man, you only have to use tasers and flashbangs, while your whole team can use the guns, so you would just need to send your Officers in, and they will do the whole job, doesn't matter if one of them gets incapacitated by the Russian Mafia/mob, as long you only use the taser and flashbangs, you will get the trophy, and it would be easier to get. ||(It just took me 30 minutes)||

While Medal of Valor could be harder, since you have to do all the maps in S tier, meaning that you have to go fully non-lethal, and if one of the suspects dies, or even the civilians by the crossfire for the suspects fault, you have to restart the operation, same if one of your officers gets shot down.

It took me 3 days to get Medal of Valor.

I blame Greased Palms, it took me 7 hours to complete ||(Multiple attempts because suspects killing civilians accidentally or officers dying in the process)||, it could only had took me 2 days

if you're trying to S rank, you can't have any suspects dead, if you're trying to get an A rank minimum, you gotta have at least a couple or more suspects alive, like maybe 3 minimum.
edit: they can't be incapacitated either; it counts the same as them being killed.

You can only get it if you damage the suspect beforehand, C2 can't incap otherwise i don't think
