If by ballistic masks you mean mandibles then yes they're already in the game. If you mean riot-style face visors, idk, maybe someday, they're already on the cop NPCs so I wouldn't be too surprised

yknow the swat ai is pretty bad but it's one of the best ais i've played with in games, like compared to players sure it's not great but at least it doesnt get stuck aggressively trying to climb ledges over and over and over and over and over
and it's hard to improve ai just in general
one aspect of the game not getting improvements isn't a sign the whole game has been thrown out

There's a few hitches still, but they are at least fast when working properly now

i think its fair to say "hey the AI Isn't really that good as how it was/how it could be and it has obvious flaws" (most notably i've had the ai just walk into traps, send stingers straight into their feet, and Drop Their Shield When Getting Shot at oh my god that is so annoying)
but, at least in my opinion, the ai isn't broken until it can't breach a room

insurgency has a much shorter TTK and higher emphasis on full auto especially in PvP so each shot is more lethal

How do I get "Way Out West" achievement for Rust Belt to proc?

I have done it solo, only the 357, never swapped weapons, all my loudout slots were for the 357, and I would only wear the shield on my back but never pull it out.

2 clears like this and no trophy pop. Shame. Any ideas as to a fix or what causes it? Can I not wear armor?

Light Armor all the way, 8 flashes, a few rifle mags, and one pistol mag. Use the G18C on semi auto to get the most out of that backup pistol since it has 24 in the mag. (Highest capacity of all pistols)

All team AI should wear heavy armor, shields, and run hard hitting guns like battlerifles and shotguns.

The shields protect them when entering and clearing but also from being shot in the back which happens frequently when they bug and overshoot doorways.

Use Horizontal Grip and SDR sight on your rifle and use your laser to aim 9 times out of 10.

The Horizontal grip increases accuracy which is the best stat of all. SDR is perfect for solving some vertical recoil and allowing for long shots when it comes to hostages / open spaces.

Shoot the groin / belly area as most vests cover upper body only.

Use Mozambique Drill with pistols (2 to the chest, 1 to the head)

Dont play Mr. Nice guy, shoot something if you deem it a threat. You only need C to pass and 1 mistake and you reset.
