You can usually tell when something looks off or has meaningless lines or incomprehensible mushes of details on it, or when something like a drawing has an overwhelming amount of detail and symmetry that a person wouldn’t do

Even when you look into it the story is still bland, uninteresting and disjointed, your only payout after interacting with it is some memorabilia that you look at once and forget it ever happened, once a storyline ends you're just flung into the next one and you don't even notice it

While it may have extensive lore if you look into it by the end of it all it all leads back into the same nothingburger, if anything RON looks as deep as a lake but is actually as shallow as a puddle

The biggest fumbling in my opinion is also the presentation, the farm 911 calls especially are hilariously corny and don't even really make sense? Why would the cult taunt the police if they want to stay off the radar?

The trailers themselves had a very clear focus on the PEOPLE of los suenos, claiming that you would run into and interact with people from all walks of life, hear and see their stories and maybe even have an impact on their lives

I mean think about it, their target audience was the very same gang we end up fighting in several missions. Chances are the very weapons they sold were used to kill other people at some point.

Theres no evidence supporting this, but for all we know the auto sears they made could have been used by the elephant shooters.

If anything I think this mission serves as a critique of the us healthcare system, had it been free, or even cheaper they very likely never would have gone to such lengths in the first place, but it still doesn't justify what they did.

Ultimately death is a natural part of life and to a degree people should respect it as such.

I absolutely agree with your point and the ultimate point to be made in that mission is that healthcare in Los Suenos (and ultimately the US) leads people to desperation due to how crippling the costs are to people and the lengths and effect some people can get to, once they're pushed up against a wall.
