It would be nice to get some strange meat every once in a while. The ring doesn't even make them a common drop, so passive is fair I would think. Besides, it barely beats out swordfish at 99 constitution.
The Surgeon's Ring Venom is talking about offers a chance to remove adrenaline loss on thresholds and special attacks.
Given the challenges you go through to unlock it, it could be an option once you've unlocked all the extra chests in the mansion, so getting the Challenging Maria achievement could give the ring and as a passive effect.

this effect is right up there next to the relentless perk in terms of stuff i hate having to adapt to in combat, 1 gdc to spend the adrenaline saved when you almost always already have another ability queued up is stupid, before this is considered for any kind of passive i want the effect changed to remove that issue
the change i've been floating in my head is when either triggers to save your adrenaline, your max adrenaline is increased by 10% for 5 seconds, that way the worst case is your next gcd can still be a fully gaining basic ability if you were full adren when asylum or relentless procs, effectively giving you 2 gcd instead of 1 to capitalize on the saved adrenaline
