The dichotomy between two world is a huge theme of omori, fiction vs reality, wallowing in guilt vs finding the courage to face your problems head on. And a huge part of that comes with the endings. It really can't get any less subtle than the good and bad ending having you follow your fake friends up vs the stranger down.
The manga, being a fully linear setup, breaks that whole deal a lot, and while they try to fix it by adding some headspace and real world in each chapter it only serves to further prove this doesn't work in any media but a game
