Joseph's blood was put into DIO's body, there's no way for them to seperate the vampiric blood from Joseph's, so when they did the transfusion, both Joseph's and DIO's would be put into his body, and he would have become a vampire due to DIO's cells, as seen in the case of Vanilla Ice.
The only way Joseph would not have become a vampire is if his blood has residual hamon left in it that fought off the vampiric cells, however, due to the blood not hurting DIO upon consumption, I believe Joseph did not have any hamon energy left when he died.

DIO’s blood doesn’t always heal or revive folks, I think it’s something he has to do consciously. I’d imagine that’s how the transfusion worked.

It wasn't said in the anime or the manga, but when Joseph was a baby, Straits coughed on him, turning him into a vampire.
I know this because I asked Araki about this exact thing and he told me this himself.
So, really, it just goes to show that you can't always believe what the anime and manga tell you.

The meta reason that we didn't see Stands or hints of them early on is because Araki hadn't come up with the idea yet.
In universe, Stands have always existed, but we're only seeing them now because the protagonists have Stands. The number of Stand users overall in the world is still relatively small, and the appearance of all these Stand users gets elaborated on later.
