1. He experimented on his mom first of all. Josuke is getting the grafted fruit to save her.
2. Exposing himself that early would be bad. What would he do to get the graft? The best thing for him was to attempt to get the branch in secret like he did at the end of Ozone Baby arc. He had more resources because of his affiliation with the hospital and more tools at his disposal.
3. Rai and Josuke are two of the most observant characters in the series, they would be suspicious of Tooru immediately. Just like when Rai was suspicious of Jobin. And in fact they WERE suspicious of him when he showed up.
