We also don't have any idea if Tooru has worked with the other Rock Doctors too

So I can assume there's a Head Doctor but there's also that stand that takes the form of the Head Doctor

The stand’s manifestation is activated by the idea of pursuing the head doctor. I won’t explain the calamity ability because that’s obvious already. But an example of this is Taoka going to the head doctor’s office and finding him standing in the corner - this is the stand and it has manifested where Taoka expected to find him first. The reason the stand had to walk to Josuke instead of just appearing near him is because Josuke is not intending to pursue the head doctor.
