Considering how single-minded the Conductor is, I wouldn't be surprised if it turned out she was originally a copy of Maya that the Vex reprogrammed to investigate the Echo, but that ended up going rogue.
the copies of Maya were sourced from when she was working at Ishtar, which is decades before the collapse, and the real Maya's discovery of and experiments with the Veil. She became obsessed with it, which (alongside the apocalypse) led to a very serious decline in her mental state

The main reason why humanity is still alive is because of The Vanguard in general
The Vanguard never intended to be in charge of the City, to be fair. They were simply a part of the Consensus, and decisions affecting the City internals were decisions they largely abstained from.
Also, sort of? They're aware of threats, but are also ignorant in some ways. The Vanguard withhold a lot of information from people.
Plus, Dead Orbit was one of the largest factions in the City before Splicer, and their entire plan was to abandon the Traveler, thinking it to be the beacon that drew threats to the solar system. Guardians kind of go hand in hand there.
Then of course some people are just stubborn.

Oryx's Throne, if you're wondering, was the Dreadnaught. He inverted it from the Ascendant Realm into real space to keep it mobile and defended, after Crota was tricked by Savathun into cutting through a dimensional barrier and allowing the Vex to invade it, causing a war only ended by Oryx's own return.
