when Act 3 for Echoes first came out, both boss encounters in Encore had Darkness zones (Omen and Tormentor darkness zone activates in the second room you fight Tormentor in)

and the Choral Mind Darkness zone remains for the entire fight

however, towards the end of Act 3, Bungie decided to remove the Darkness zones in both bossfights for the normal version of Encore

it is worth noting that the Omen and Tormentor fight had a similar issue in the parts before the Darkness zone activated, so this happening in the Choral Mind fight too when its Darkness zone got removed isn't really surprising in hindsight

On top of that, even when Osiris had a straight up vision of Neomuna, the Hidden scoured Neptune and didn’t find it

The Vex had done some shenanigans to hide the City. It was only revealed thanks to the Witness exposing it and sending its fleet there
