I believe i got this trophy after multiple play throughs perhaps if you play through the whole story once and then help a friend or a stranger get through the story you should find enough

Its trial and error, this achievement is also known to glitch a ton. My tip is to finish the game and then start a new one as encounters will save across any new game. Keep doing this until you get all 15

No guarantee on this as haven't tried myself, but if you are able to find a hostage encounter.... complete it fully wait a few seconds then fully back out of the game and then see if it spawns in the same location...if it does just keep grinding it until completion. Another thing I found is that you need to complete the encounter 15 times rather than save 15 people as I saved 2 hostages and it only went up by 1.
