if you have your hudon you have to be getting chased for a certain amount of time / fighting them for an extended period without them losing sight of you

theres is a bar with 4-5 notches on it that shows chase level ( i think it only goes up to 5 on nightmare)

There's a trophy for giving 7 sectors to 1 faction, and a trophy for assigning all facilities but you can get both done in 1 playthrough. Only trophies that would really require more than 1 playthrough is maxing out health and stamina because NG+ adds more of the item you use to increase those and allows you to have both maxed out after collecting them all.
