If there is a sequel it’ll probably just take place after the first but there’s just so much stuff they could do with this universe

The secret ending of BB has you become a great one and kill the Moon Presence, which is the being responsible for the Nightmare. By that point, there's not really any looming threat anymore. Even with the DLC, you kill the Orphan of Kos and free all hunters from their curse. This is a Far Cry from the first Dark Souls, where you (probably) kindle the flame, only for it to eventually fade once again, and the DLC is irrelevant in the big picture.
