Yeah, I tried on two Mikes. One with armour and taking away maybe 40% of his health due to shrapnel. It never did the thing.
Second Mike I removed armour first, tried super molotovs and then also burning throwing stars until about 40% life dealt that also never did the thing.

I think it's more of a chance thing. The ailments needs to be active at the same time, and there's only 1 explosion from the grenade/trap and multiple chances from stars/molotovs.

And they dont always do those things.
