Death messages from /damage are broken. /damage @p 25 minecraft:arrow at ~ ~ ~ and you get a messed up message
In singleplayer it will show the translation key, but still better than what Paper does

Okay: Death message seems to be with %2$s in vanilla too, if you are not targeted by an entity. If you are targeted by an entity, it shows the translation key instead.
