I had the same thing happen. I loaded from scene and had the same thing happen again. Here is a clip of the bug.

Hmm. Really sorry this happens. Does a reset using the chapter select work for you? I will log this one.

Tried 7 times from reset , im done till theres a patch

Just want to update you on this. There is an update out that fixes this for Steam, the update is on it's way to Xbox. It is in certification and should be live early this week.
