Take the old one and draw over it.

You can also use Go Dump and Sprite Packer. Go Dump generates individual png files, Sprite Packer will put them back as they should be.

It should be possible - you need to get every randomized check (including flower quest and colosseum 1 and 2) and buy out all 5 shops (the shops won't appear in the helper log if you've bought some but not all the things there), and in area/room rando you need to pass through every randomized transition.

Yes, but
A. You have to find someone who's willing to take a commission for it, and
B. You should plan on it being a pretty pricey commission ($$$). There are a ton of sprites that need changed for a full skin. Even if you just want the basic knight done, there's hundreds of sprites that need edited / created.

I know this question is probably asked a lot, but what mods should I try first?

I'm already aware of skins, but I'm wondering if there are mods that add new areas/bosses or some or other new content

Randomizer 3.0 is one of the best mods. It still requires playing on 1.4.3.2 (it's not on Scarab yet), but it'll probably always be the "best" gameplay experience mod.

Silksong ᵇᵘᵗ ⁿᵒᵗ ʳᵉᵃˡˡʸ is great if you want all of the fun of role-playing as Hornet.

HKMP and being connected to a public server is nice to leave on if you want to randomly run into other players while doing other stuff.
