the less realistic part is the natural consequence of stripping away parts of the environment that arent crucial to the game experience
the max graphics settings are the ideal experience, lower settings are there to take away small parts if you cant run the full thing
its just that in most 3d games(that naturally emulate more of the real world than 2d can) its more easy to notice that something isnt like in real life because we subconsciously compare the virtual 3d space to ours

That area is called The Farlands. Maybe you're right, after the Lace Fight, after we exit the bell chamber, we do enter some kind of area with a similar vibe and nature to it, although when we do exit the bell chamber, a sign with a city is displayed, so unless there is a city in the Farlands, I don't think this is it.
