A couple things to check
- (I honestly don't think this applies to 1.5 modding but better to check) From the title screen, open the options menu and check for a "Mods" tab.
- Check that it doesn't say "Enable API" in the bottom left of scarab, if it says "Disable API" it's fine, also try installing another mod real quick.
- Press "Open Mods" in the bottom left of Scarab, this'll open your Mods folder in your game files, then assuming you're using Steam, you'll want to get to your game files (Steam library > right click HK > properties > local files > browse) you'll then want to compare the file paths of the two folders, as it's possible Scarab is looking at the wrong one.

Tried it on an existing save it worked fine, tried it on a new save, first time it doesn't fire a projectile but the rest works, muzzle flash and no pauses. Just works like an ak47, reload save and it works just fine

If so, there's god soul and hell mod (is hell mod updated yet?) available that make the game much harder if you'd like to try those. Or if you're looking for difficulty there's also several modded bosses which are very hard, such as inferno king grimm, any radiance, pale prince, etc.
