(I think the issue comes if your last ||entry is one that gives you the notes as the same time as the basic entry, ie you go from 140/141 ; 140/141 straight to 141/141 ; 141/141 and so the game only gives you the achiev for getting all the notes, and not the one for getting all the entry, absolutely not sure about that)||

If you are on PC, you may wanna try a few ||mods, some in particular will let you for example play the climb on repeat, which can be really useful||

If not then there are a few basics tips, but in the end it comes to getting down some sort of intuition

What I do, but many will disagree is

• ||jump when I see the telegraph for a laser, that way the beam is gone when I am airborne||
• ||when you get close to the top I tend to do very large jumps to the side in order to bait the lasers very far away and I dash back to the platforms using shade cloak||

what can cause you to fail is if you dash to early, the one thing which is kinda confusing is that first telegraphs of the actual daggers appear, don't dash when you see those, dash when you see the actual daggers

as for the lunge (and the triple slash) I dodge both by simply jumping above PV and pogoing them
in order to do that consistently I jump whenever they are about to start an attack, as being in the air is well, the first step towards pogoing them||

personally I rage quitted the last % for a couple of month
in the meantime I got all other achievs (including ||steel soul|| which isn't as hard as it seems) and then went on to try mods (even though I had never modded a game ever before)
and after giving the game some extra 30 hours I came back to get 112%

I would really recommend doing that over grinding ||pantheons or the hall of gods ||for hours, which simply isn't fun
and just playing will make those endgame challenges easier

so i got myself a ||"cracked"|| version of HK and for some reason my achivements are no where to be seen in the menu options even the unclaimed ones like ||steel heart|| are not there. there is just nothing! For example, ||after gettting the Dream No More ending and beating p1,2,3 and 4 ||the in-game pop ups were there but when i checked the menu options they list was completely empty. super super lategame spoilers ||I cannot consistently get the whole "oh just pogo off her head and the orbs despawn!!!!" thing to work ever for absrad's last phase. Literally has not worked even a single time for me even when I'm so far offscreen I can't even see a pixel of the knight. It gets me killed every time. Is there an actual strat for this phase that isn't reliant on an exploit that doesn't even work or am I just screwed lmao||

||thats generally not the strat you wanna use anyway but if you do use it, you want to pogo when you can see the knight on screen. however as you mentioned it can be tricky. the alternative (and imo better strat) is to start the phase by going to the left platform and then instantly shrieking twice (or just once if you dont have enough soul). if you do shriek twice, be ready to dash to avoid the orb, at this point the final phase will end in just a few nail hits.

if you dont do any shrieks, then stay on the left platform and do side slashes/pogo radiance when she comes by u and use fireball when shes on the opposite side of the arena.

the last strat is to jump off to the left in a circle motion and constantly use dash slash, it can be a bit tricky, however its consistent to the point that u could easily do the phase with your eyes closed given u practiced the movement enough ||
