Lore time:

In a shaggy old backwater near a river, disease was common. But one terrible night, a new scourge arrived. The river began swarming with large, dark leeches, latching onto anything that enter the water. Fish were fished up covered in them and dead. Men who even dipped their feet into the water were instantly attacked by several of these monsters.
And those who had been bitter, were cursed with a terrible affliction.
Their skin became greenish and slimy, they began hungering for blood.
Their hunger was relentless, always present. They drank the life fluid of any poor creature that came across their way, and as they downed the saline red fluid, their intestines expanded and swelled rapidly, takin on squirming, wrinkly, slimy leech-like looks. Their guts swelled with immense amounts of drained blood, splitting from beneath their skin.
They wanted blood and only blood, starving themselves of real nutrients and eventually succumbing to either malnutrition or their bloated wounds.
