"qu'ai-je fait ?" is the formal variation using inversion
all three sentences are grammatically correct, using the three ways of asking questions in French (intonation, est-ce que, inversion)

your sentence is informal. the est-ce que variation a bit less so, still used casually.
inversion is very formal and never used casually

I feel like you wouldn't use "j'ai fait quoi" for surprise at doing something unintended (oh god what have I done) but if you're asking what you did to upset someone for example it could work imo
"Qu'est-ce que j'ai fait?" Could work for the surprised example though
