As you can see here, the sentence makes it pretty clear it's talking about one person, Marie, so it's « elle ». « lui » here stands for « à elle (to her) ». If it had been plural (as in we're talking about multiple women), it would've been « le médecin leur a dit »

In other cases, French has this phenomenon where it stresses/emphasises a noun by repeating it in its pronoun form.
« Salut, tu les as vues, Marie et Jeanne ?
– Non, pourquoi ?
Les filles, elles mangent une orange !
– Du coup… ?
– Le médecin leur a dit de pas le faire, putain ! »

2nd conjugation (-ir): Il finit / Ils finissent | Il dort / Ils dorment | Il vient / Ils viennent
3rd conjugation (-re): Il rend / Ils rendent | Il conduit / Ils conduisent | Il atteint / Ils atteignent
Irregulars (-oir): Il peut / Ils peuvent | Il sait / Ils savent | Il reçoit / Ils reçoivent
