Make it descriptive, including relevant context, but also to the point. This way you improve your chances of getting a more relevant and specific answer.

"Que" in a question would be used to indicate an object, so it'd be "Que se passe-t-il ici?"

It can't indicate a subject for questions. For example, I can't say "Que marche?"

Funnily enough, « que » can only indicate a subject with « est-ce qui » as you've said

French Wiktionary has this table which shows « qu'est-ce qui » as the only valid way to have « que » as a subject

Therefore, to use « se passer » with « que », you either have to put « est-ce qui » or use it impersonally with the pronoun « il » for the whole thing to work

I also do want to note that you cannot ask a question with « Que » at the start without inversion. Because it has the form « Quoi » placed at the end of the utterance, « Que » tends to be placed at the end of a phrase to become « Quoi ».

This doesn't happen with the other question words like « pourquoi » :
« Pourquoi le fais-tu ? »
« Pourquoi est-ce que tu le fais ? »
« Pourquoi tu le fais ? / Tu le fais pourquoi ? »

For comparison, « que/quoi » is like this :
« Que fais-tu ? »
« Qu'est-ce que tu fais ? »
« Tu fais quoi ? »
Here, « *Que tu fais ? » cannot exist.
