"Tu amputes quelqu'un" si tu es le médecin et qu'il faut couper.
"Tu t'ampute une jambe" si tu as pas le choix et que tu es seul sur une île.

Amputer is medical word most of the time
s'amputer doesn't really exist, but could still happen because you can use "amputer" transitively
"amputer [quelqu'un]" is "to cut [someone]'s limb"
so if someone is cutting their own member, you can use this verb reflexively
also yeah it usually describes the medical procedure
yeah similar to English, honestly amputate really just is to cut off specific parts of the body
