La préposition « à » décrit la manière dont quelque chose est fait donc en disant « blessée au couteau », on dit qu’une blessure est créée à cause d’un couteau. La préposition « de », en revanche, décrirait une possession comme si le couteau contient ou possède une blessure.

You could also say "blessé(e) d'un couteau" (not de couteau), though? It seems that with de + article, the cause of the injury is introduced, but with à... either the target or the cause are both possible? blesser quelqu'un à la tête / blesser quelqu'un à coups de couteau, hein ?
