If you use connaître can you change the meaning to something like "do you know (personally, are familiar with, are acquainted with...) the person who wrote this?" Or do you have to rephrase somehow to get that meaning?

If you were asking not about knoweldge of who it was that wrote it but instead whether I personally knew the person who wrote it, you could ask:
Tu connais la personne qui l'a écrit ?

this isn’t definitive but knowledge that exists in an independent clause is always savoir

so you say “je connais la personne qui l’a écrit” but “je sais qui est la personne qui l’a ecrit”
