il est un médecin -> design his profession in detail (you can add "il est un médecin urgentiste" for exemple)
But when you say "un" it's more correct if you say "C'est un médecin"

when you are trying to abbreviate "you are" you say "you're", not "your"
"your" is possessive (example: "Your dog is nice")
so it would be "you're welcome" because it's like saying "you are welcome"
