i think it depends on size of biome , maybe too small so not many show up at once or too big and that mist of them spawn in a hole beneath the biome.

yeah mobs cant spawn on lava, they spawn near the lava but not on lava and i think you need to be 45 blocks up for you to start spawning monsters i could be wrong tho.
