I feel like when in stressful situations, I focus more on threats to me than anything else. In Enter the Gungeon, I have clutched more often in bosses than not. With full health, I get to cocky and focus more on dealing damage than really dodging. When one hit away from death, I see all my threats, move extremely precisely and almost ignore damaging. In my first Moon Lord fight, there was a point where I was 40 seconds away from a heal and at 60 health. Something clicked.
