I had this issue happen and to resolve it, I exited to main menu and reloaded the save. I think its just an inventory menu glitch with item interaction.

Perhaps it would be beneficial to allow for asigning items to specificly off hand or main hand. Because I have this problem every time I switch weapons. I want my shield to off hand but everytime it goes main hand.

@celest path if you assign weapons in the abilities tab you can choose whether the hotkey is for main or off hand.
cant see my pointer in the caps but in the first i am hovering abilities tab on the left, second: over the 5th slot, labeled "2" third: over main hand.

I use a controller myself and i find it's pretty okay for the most part. for the main hand/offhand equips you have to hit whatever button's mapped to 'Item Menu' and select it there or map it to a quickslot if you do a lot of swapping around
otherwise it defaults equiping things to your main hand

Just tried it with controller, works fine. do you want me to send vid tutorial? controller ui is a bit jank, but usable.
one of the biggest issues i find with controller is that directional pad doesnt work well in most menus

It's functioning just fine for me, you may be experiencing a bug. I have no fix for that, sorry.

You are showing me two seperate weapons. I'm attempting to get my shield to go into my off hand. Every time I use one of the assigned items it goes into the main hand. Shield and all friend.

gotcha, i'll try it with a shield, i've noticed the have some weird interactions with weapons. which shield are you using and which weapons?
you may also want to cap a video, it sounds like a UI bug.

Yeah Brother, I've tested 7 different shields with a dagger, axe and pistol. and aside from one that was oriented incorrectly its working fine here. you've either got an injured hand, a UI bug, or some combination of both. I can't reproduce your conflict.

I had this during Tutorial (was not able take Mace with Shield).
But after started actually playing I had Dagger and Shield working
