You said you ran lp user <your username> permission set true, the wiki says lp user <your username> permission set luckperms.* true. You missed the luckperms.* part, the actual permission you're trying to give

Has LuckPerms been exploited/hacked?

Most certainly: no. In most cases a 'LuckPerms exploit' is a simple case of human error, which can be avoided easily. Look below for some tips to be on the safe side.

Don't give random people full ( * ) permissions / admin permissions

While this should be obvious, a lot of times a exploit can be traced back to faulty permission settings. You should always take your time with permissions and read the plugin documentations. In 99% of all cases the plugins have documentations explaining each permission, in the rare case that the plugin you are using does not have a documentation, you could still:
A: Ask the plugin developer for help
B: Use LuckPerms verbose functionality ( !verbose )
C: Select a different plugin with proper documentation.

Do not run your server/network in offline mode

If you are running your server or network in offline mode, hackers have it really easy to steal your, or any other admins identity. While the server is in offline mode, certain checks ( which exist to prevent exactly this ), are being skipped, and the server does not verify if the person joining actually is the person they claim to be. While there may be plugins which increase the security of offline mode servers by adding things such as admin codes, you should just switch it to online to prevent the issue in the first place.

Do not download plugins from shady websites / sent by friends

You should never put anything on your server which has not been downloaded by yourself from official sources. Plugins can be infected with malware which injects itself into all other plugins, and thus is hard to remove. While it may look like a plugin has been hacked, you most certainly downloaded a modified version of it and it is not the plugin authors fault. In case your server has been infected by such malware:

  1. Stop the server
  2. Delete all plugins in your plugins folder, and just to be safe also the server jar file
  3. Re-Download all plugins and server jar files from official websites such as SpigotMC or official plugin websites ( luckperms.net for example ).
  4. Check if there have been any modifications to the permission system and remove unknown users and wrong permissions.
    if you follow these steps you should have a clean server by the end, without exploits.

Remove Spawn, serverhomes, simple RTP, TPA, and Vanish plugins and use an essentials, such as essentialsx to do all of that and more in a single plugin

Just a tip to you, some of my top recommendations since you're new to Servers

  1. If you want to use bukkit/spigot - Use Paper it's so much better
  2. Top Recommended plugins are: Luckperms, EssentialsX, Vault, PlaceholderAPI, WorldEdit, CoreProtect, WorldGuard
  3. Don't rely on op, focus on getting a sound permission setup and you won't need it.

Has LuckPerms been exploited/hacked?

Most certainly: no. In most cases a 'LuckPerms exploit' is a simple case of human error, which can be avoided easily. Look below for some tips to be on the safe side.

Don't give random people full ( * ) permissions / admin permissions

While this should be obvious, a lot of times an exploit can be traced back to faulty permission settings. You should always take your time with permissions and read the plugin documentations. In 99% of all cases, the plugins have documentation explaining each permission. In the rare case that the plugin you are using does not have documentation, you could still:
A: Ask the plugin developer for help
B: Use LuckPerms verbose functionality ( !verbose )
C: Select a different plugin with proper documentation.

Do not run your server/network in offline mode

If you are running your server or network in offline mode, hackers have it really easy to steal your or any other admin's identity. While the server is in offline mode, certain checks ( which exist to prevent exactly this ) are being skipped, and the server does not verify if the person joining actually is the person they claim to be. While there may be plugins that increase the security of offline mode servers by adding things such as admin codes, you should just switch it to online to prevent the issue in the first place.

Do not download plugins from shady websites / sent by friends

You should never put anything on your server that has not been downloaded by yourself from official sources. Plugins can be infected with malware that injects itself into all other plugins and thus is hard to remove. While it may look like a plugin has been hacked, you most certainly downloaded a modified version of it, and it is not the plugin author's fault. In case your server has been infected by such malware:

  1. Stop the server
  2. Delete all plugins in your plugins folder, and just to be safe, also the server JAR file.
  3. Re-download all plugins and server JAR files from official websites such as SpigotMC or official plugin websites ( for example: luckperms.net ).
  4. Check if there have been any modifications to the permission system and remove unknown users and wrong permissions.
    If you follow these steps, you should have a clean server by the end, without exploits.

This is correct, in general LP should not require an update to support new MC versions on specifically Paper / Spigot or forks thereof.
Sometimes there can be some quirks with click & hover events (i.e. /lp editor links not being clickable) on the new version, but this is a cosmetic bug only & is generally quick to be updated. If you encounter this, the link itself is still good & can be manually typed into your browser

That API is what allowed a native Forge version of LuckPerms to be made, however that API does not currently exist on older versions of Forge, and is unlikely to be backported. If you don't want to use particular features, not using them means they have 0 impact on your server's experience or performance. Essentials also allows you to set the "no permission" command to whatever you want, so you can set it to the vanilla "Command not found" line

My Users fail to load with a local storage type of H2 and SQLite. I've tried fully wiping and clean installing lp and it still has the same issue. Changing the "use-server-uuid-cache" value to true or false doesn't change anything. Other than those two settings all other settings i've left default.

I know of tmpspec, it functions great except for the part where it doesn't have any integration with luckyperms. So everybody has the perms to do it.

Try Banhammer, It allows for tracking of each users infractions and allows you to tempmute, tempban, It also has auto punish functions in its config (I dont use those) To pair this use Invview, it allows you to view players inventory's and Echests, allows grabing peoples items from the inventory aswell, without using a tool like, NBTeditor. Though the inventory grabbing feature can be turned off, or managed through lp.

Both of these mods have intigration with luckyperms so you can make sure that only the mods or trusted people have commands.

Additionally, if you want the group/user to be able to promote using global context (without requiring certain contextual conditions), you need to add the node luckperms.usecontext.global.

LuckPerms does not perform any chat/tablist formatting of its own!

LuckPerms only acts as the source for prefixes / suffixes, it doesn't actually apply them. You need an appropriately configured chat/tablist formatting plugin for them to be displayed. If the values appear correctly when you run /lp user <user> info, LuckPerms is doing its job!

How-to test permissions

Small list of things for you to make sure when testing permissions:
1. Don't test as op, your players/general staff will most likely not have op
2. Make sure you don't have any special permissions (e.g. * or the same permission you are testing) on your user (/lp user <user> permission info)
3. Make sure your parent groups are the ones the target audience you are testing will have (if you are testing mod perms, don't have owner as parent)
4. Use verbose to see exactly what's going on, what permissions are being checked
