I have a theory ||that though they are nightmares they definitely have some sense of reality to them. Chapter 2 is seemingly a dream but his actions influence reality somehow. The whole first section the cult is messing with his head and convincing him he’s a killer and delusional, then Garcia comes out of nowhere and helps him break through their lies, reassuring him that what he believes is true. That’s when the cult stops playing with his head and Miriam attacks.
This is further suggested by the fact that if John chooses to fall for their manipulation and lose faith by following the unspeakable, though he has done nothing in reality, he wakes up and is blessed by it. Garcia’s note is gone. ||
