yeah, we dont really do anything that systematically, we just go with what feels right
the enemies do lots of different things so we consider how they interact instead of just a flat difficulty level for each one

I've just started (past 4 months or so) creating levels - Been making a few for Overload and got some into the upcoming community DLC. Gotta say, I've gained a whole new respect for level designers. It is not easy, and it is time consuming, at least in my experience. Of course, I've also never used any other 3D modeling program or the like, so it's also been a huge learning experience.
The Overload editor is a cube based system (turned out to be harder to make the shit I wanted to make than expected). Not sure what AE uses, but that's not really important in the grand scheme. It is, as mentioned above, a lot of "does this feel good to play". You can have a totally kickass idea on paper that sucks to play in. I've spent hours and hours (and hours and hours) tinkering with my levels.
I'm just starting, but its been really eye opening. I highly recommend picking up a game that interests you and has a level editor just to tinker around in.
Although I will warn that such actions may consume every bit of your free time lol

ROTT13's levels were actually based upon the original maps (believe it or not)

So just browsing in the steam reviews/forums and playing again, I can't help but think that the 100% chance of chain lightning on overkill with the trident does feel kinda OP, as suggested by user Wireframe. As nerfing the damage or the aoe would take away from its awesomeness, nerfing the chance of chain lightning triggering on overkill would possibly contribute to weapon balancing quite effectively, making the experience a bit smoother overall?

i think it works fine
you have to keep fire held on an enemy for a good second or two after they die to get the chain lightning
and that can be challenging to do when also moving around and dodging enemy projectiles
so i think its balanced
