I've done a little testing, my conclusions:

1. The timer does increase if you are loaded into the world but pause the game with the menu, so if you pause the game to go and get a snack or something that's time added to the playtime counter without in-game time progressing.
2. If you tab out (alt+tab) the timer does not increase, regardless of whether or not you pause before doing so; this mightn't be the case if you have a mod that keeps the game running when it loses focus, but this isn't intrinsic to base cal so it is irrelevant.
