It gives damage to any ability that spends mana. Some classes for instance do not spend mana on T or X skill and thus can't really utilize MP efficiency very well. Awakening doesn't cost mana as another example. MP efficiency is a comfy choice since it alleviates mana problems and doesn't have the heal penalty.

You’re looking at outdated info, the engraving hasn’t been like that for a long time. The engraving in game tells you that it’s for anything that spends mana. The damage difference is like ~1-2% damage overall or something relatively marginal (I guess ~10% if you’re trying to directly compare engravings?). If you can get some relic points on mp efficiency it becomes better than no relic point cd since cd is expensive. Personally I run mp efficiency if my T and X skills cost mana on dps classes.

After checking the roster, I probably could have answered my own question. Every character uses grudge but one doesn't use adrenaline in the recommended build. So grudge is almost certainly the most useful for me right now.
