Please run Syscheck and test all IOS on your Wii by following this guide:
https://wii.guide/syscheck
When it's done it will generate and display a syscheck.csv file.
If you don't have an SD, you will get an error. Just continue until the report is displayed. You will need to upload photos of the entire report.
If you have an SD, power down the Wii and insert the SD into a PC. Drag and drop the syscheck.csv file from the root of your SD into chat.

Your cIOS on slot 249 is not correct.
Follow this guide to install cIOS on slot 249: https://wii.guide/cios
Your cIOS on slots 250 and 251 are fine. Just follow the instructions for slot 249 to overwrite your incorrect cIOS
