Well "down" can be many different things. In this case for example, it's only some features that are down, and not others, at least on our shard: for example, auto responders still worked, a dozen minutes ago, while autoroles have been broken for almost an hour now on our end.

I don't know all the modules (as we only use a fraction of them), but in theory I think a lot of them could be automatically tested. For example, the bot could try to give itself some dummy role, add some reactions to its own diagnostic message, etc. etc.
Like you said, probably couldn't be possible for everything, but even partially it'd still be useful. Because right now the issue is, you use ?ping, it says something that looks okay, and then you're stuck waiting to get more official status updates, without knowing if it's something that broke on your end (say, a mod did a config change and broke something), if it's just a temporary hiccup, or if it's a more global outage.
