I had done S+ twice was happy to get my third one to have the whole outfit and we’ll I went for another run finished it with 2deaths and I didn’t get S+ item I got a a rank item so could someone help me get it

S+ in what? I assume you mean Crescendo? Then only die 1 time. It also depends on when you die as it affects ths score differently. For instance, you die in act 1 at the start, you lose points, but you didn't have that many points to lose, so it would not affect it as bad. But, later, when you have more points, it take more.
It acts just like the other quests, but you can't see the score. So, for instance, you lose 1500 points on a death or 2000 points on a friendly fire, you die at the start and only have 500 points, you only lose 500 points. It does not go below 0. But, later, after killing more things, you lose the full amount.
